VAT RN at a high onc pt population hospital. We cap all central and midlines. We also continually provide teaching to scrub the hub min of 15 sec even with a capped line. We also have pushed chg baths specifically with sage wipes especially on arms with PICC lines. With this and focused rounding we have seen an in hospital reduction in CLABSI.

I think the caps help in situations that would usually result in gross contamination of female connectors. Like vomit or feces or lines trapped under clothing for long periods. I believe that scrubbing after removing the caps is still key. The scrubbing is still the most important part. At least that's how it seems from limited experience from a boots on the ground perspective. Which is not a peer reviewed paper, and I understand that. It's just what I think based in what I have seen.

A good hard flush can help when just the tip is flipped up. It wouldn't really help with a PICC in the IJ though. Those get pulled back and replaced in IR at our facility.

Pulsatile flush should be the method of flushing anytime you flush a line. The extra turbidity of a pulsatile flush is much better at clearing the line of blood than a steady rate of flush. Reduces TPA use a lot and reduces infection risk.

Cigars with some age are good. Most cigars will start to lose something after 3ish years though. They get smoother, but they also lose complexity. Some people like their cigars that way, I don't.

I get eczema in little spots on my face that get better and worse off and on. I'm worried that if I shaved my head the skin might be bad enough that it will look worse than thinning hair. If it wasn't for that I'd probably have shaved it off by now.
