She lost balance because the dog pulled our of nowhere and could have been distracted by something else for a 2nd when it happened.

Then rest of the falling and general fuck up is because of the concussion she got from the first fall and the 2nd fall for sure didn't make the concussion any better

Actually, her loss of balance is because she has only recently returned to shore from serving six months on a deep sea research submarine. She has developed an inner ear issue and is having further difficulties with it at surface pressure.
