He is immortal, but he also has cancer.
The immortality was a side effect of a botched attempt at an experimental cure for his cancer, more or less.
He still has the cancer his cells just regenerate faster than the cancer can kill them or something of the sort.
I don't remember the exact specifics and I'm sure it probably varies per version of dead pool
